When he was wrong about the problem
Every fix starts with a fresh look at your live site. If the problem he planned to fix is not there, he changes nothing and records that he was wrong about it. That is counted as something that did not work, not as something you turned down.
When a fix does not check out
After a fix goes live, Bernard checks that it is there and that nothing else on your site changed. If the fix is missing, or anything else changed, the fix is marked as needing attention and Bernard puts your pages back as they were, then publishes that. He tells you what happened in either case.
When he cannot tell
If Bernard cannot confirm whether a change went live, he does not guess and he does not call it done. It is marked for a person to check.
Why it makes him more careful

- What if the problem Bernard found was not really there?
- He looks at your live site again right before he acts. If the problem has gone, or was never there, he changes nothing and records it as “Bernard was wrong about this”, rather than dropping it quietly.
- What if a fix fails its check?
- It is never left marked as done. It is marked as needing attention, with the reason, and because routine fixes are easy to put back, Bernard puts it back on your live site by himself.
- What if he cannot put it back safely?
- If the page has changed since his fix, or you have unpublished changes of your own, he does not force it. The fix stays marked as needing attention for you to decide, and you can restore the earlier version from your site history, just as with any change you approved.
- Does one mistake change what he does next?
- Yes. A fix that changed something it should not have stops that kind of fix running on its own at once. It goes back to being prepared for your approval until it is looked at.
